PATIENT ACTIVATION MEASURE (PAM)
The PAM, a commercially licenced tool, measures the spectrum of skills, knowledge and confidence of individuals. These scores can assist in predicting future emergency department visits, hospital admissions and readmissions, medication compliance and clinical outcomes.
This event will:
•explore the concept and measurement of patient activation, including the development of the Patient Activation Measure (PAM)
•discuss the use of PAM in population health management and quality improvement
•present early findings from research conducted locally using PAM
•share lessons learnt from utilisation of the PAM within a local context.
