The Centre for Primary Health Care and Equity, UNSW in collaboration with Sydney Local Health District (SLHD), South Eastern Sydney Local Health District (SESLHD) and Central and Eastern Sydney Primary Health Network (CESPHN) invite you to attend our one and a half-hour webinar.

The aim of the webinar is to: highlight research findings from the CES Primary and Community Heath Cohort/Resource (CES-P&CH) to improve policy and practice, and to explore opportunities or activities to improve coordination of care and reduce isolation among people aged 45 years and over who are socially
isolated.

Purpose of the webinar: 

  • Disseminate research findings from the Central and Eastern Sydney (CES) primary and community health research cohort.
  • Explore ways to translate the research findings to improve policy and practice. 
  • Explore opportunities or activities to improve coordination of care and reduce isolation among people aged 45 years and over who are socially isolated.
