Despite variation between national contexts, there is remarkable similarity in new models of primary care being considered across the globe. Common elements include - designated populations of about 30-50,000; inter-professional teams with new roles to complement traditional disciplines; stratification of need to enable targeted responses; and an emphasis on self-management and access of community-based resources. Achieving such transformation in practice is a complex process that involves addressing professional interests and established cultures. This seminar will draw on a primary care innovation programme in Wales to outline the challenges, and identify local and international learning about how to undertake such transformation.
