Implementing the Patient Activation Measure (PAM) to improve quality of  care for patients with chronic conditions

One day workshop for clinicians  from SESLHD  who are to pilot test the implementation of the PAM as part of their clinical practice using a ‘Breakthrough Collaboratives’ methodology developed by the IHI. The PAM aims to assess an individual’s  level of confidence, knowledge and skills and in this pilot study will  used to tailor care and self-management support for patients with chronic conditions.
